Parts A & B
Each Scout must have Medical Form Parts A and B on file with Troop 984. We need this for each Scout so that we have the general medical/contact information available for all activities should we need it. Parents that participate in Scouting activities should also fill out Parts A and B. We keep your medical information on file, but you must submit a new form
every year to the Troop as it is only good for 12 calendar months.
Part C
Medical Form Part C is for events that last longer than 72 consecutive hours, SUMMER CAMP, high-adventure activities and any activity that is strenuous in nature.
A typical 3 day weekend that starts at 5:30 p.m., on Friday and ends at 11:00 a.m., on Sunday DOES NOT fit into this category. In this instance, only Parts A & B are mandatory. However,
it is in your best interest, as a parent, to have Part C on file with the Troop. Should something happen to your son while we are camping, this form allows any Doctor to treat him more efficiently.
Note: Part C must be signed by a Doctor and is required for all summer camps. Each High-Adventure Camp has similar, but separate, Forms A, B, and C.
Individual Scout Medication Form
This form should be used for every weekend camp out where your son will be taking medication. Turn it in to the appointed Medicine Man in a Ziploc bag with the medication when dropping your scout off to leave.
A note on health form expiration from the national website:
Q. What is meant by "Annual"?
A. An AHMR is valid through the end of the
12th month from the date it was administered by your medical provider.
For example, a physical administered March 3, 2014, would be valid until
March 31, 2015. The AHMR in use before March 1, 2014, will be valid
only until April 1, 2015, and only if it was completed before April 1,
2014.
